1. Program Eligibility


Eligibility Criteria: Subject to program restrictions and terms and conditions, the Omnipod DASH 30-Day Free Trial Program (the “Program”) is open to patients who have a valid prescription for Omnipod DASH and who have commercial or private insurance, including plans available through state and federal health insurance stock market. The program is only open to new Pod Therapy patients coming from multiple daily injections or tube pumps who have not previously used the Omnipod® 5, Omnipod DASH or Omnipod Insulin Management System.


This offer does not apply to participants whose Omnipod DASH prescription is covered in whole or in part by Medicare, Medicaid, or other federal or state programs, or where prohibited by law. Enrollees in some commercial insurance plans may not be eligible. This offer is valid in the United States, Puerto Rico and US territories only. Contact Insulet Customer Support at 1-800-591-3455 for details.


2. Program details


With this program, participants can qualify for a free 30-day supply of Omnipod DASH products. Eligible participants have two (2) options based on the following:


• Participant signs a 30-day confirmation of a free trial of Omnipod DASH through the applicable platform provided by the pharmacy partner Insulet.

• Once Insulet or its pharmacy partner receives an application, including a valid prescription for the Omnipod DASH Intro Kit and Omnipod DASH Pods, Insulet or its pharmacy partner will review the participant's benefits.

  • If the benefit review results in a monthly copay equal to or less than two hundred dollars ($200), then Insulet or its pharmacy partner will issue a one-time copay card to the enrollee equal to the out-of-pocket cost the enrollee would have paid for the 30-day the initial shipment of Omnipod DASH in accordance with Section 3 below.

  • If a benefit check results in a payment greater than Two Hundred Dollars ($200), Insulet or its authorized partners will arrange for the shipment of one (1) Omnipod DASH Intro Kit in accordance with Section 4 below.

  • For purposes of clarity, the term “copay” shall include any out-of-pocket costs for a one (1) month supply of pods, including any deductibles, copays and other out-of-pocket expenses that the Participant would be required to pay. to pay for the purpose of procuring the said supply of pods.

  • Any repayment assistance may not apply to a participant's health plan deductible if prohibited by state law or the health plan.

  About lancets A lancet is a small plastic device that holds the needle securely. They come in a package with a cover over the tip of the needle, which needs to be removed before use. Once you insert the lancet into the collection pen, there is a safety cap to keep the needle sterile and help prevent accidental injections. Lancet needles come in different widths, which are measured in gauges (G). The finer and narrower the needle, the less painful it is to use. Interestingly, higher numbers indicate finer and narrower needles, while lower numbers indicate heavier and thicker needles. So the 33G lancet is thinner and more desirable than the 30G lancet .
